I got mine working by using the following structure,
Call of Duty - Black Ops 3\usermaps\zm_nuketown_hd\zone
Extract mod to zone folder
Open workshop.json inside zone folder, you should see "FolderName": That should be "zm_nuketown_hd"
Change the "Thumbnail": path to the mods thumbnail. Mine...
