Cheat - Uses reversed code from a game in order to make the client (yourself) see visuals based on where a player is on your screen (ESP), or update your crosshairs based on that (Aimbot)
CS:GO *Cheat*
A Non-Host Black Ops II *Cheat*

Mod - Uses pre-existing code, usually dumped or released rather than reversed, and modifying the way that code is called in order to modify the game.
A Minecraft Mod
A GSC Mod Menu

Hack - A method of “hacking” into a database or server and dumping account information, or edit statistics directly. Could also be hacking their servers, including DDoS, SQL Injection (if they’re that silly) or bruteforce.
Websites Account Dumps
Website defacing by bruteforcing their server

Actually we, CabConModding, use the words cheat, hack and mod a bit different.

Cheats
Provided by the game developer, fair: not online or offensive against other players

Hacks

Not provided by the game developer, unfair: mostly online

Mods
Opportunity provided by the game developers but mods actually coded by modders (not game developers), fair: gives the game more features, maps and stuff

Well you're using incorrect terms... Cheat's aren't fair, Cheats are used online and are offensive to other players.

Hacks require "hacking" skills, not cheat development.

Mods is modifying game code.
